Trusova INTRODUCING OPTIMAL METHODS FOR UMBILICAL CORD BLOOD PROCESSING AND TESTING To study optimal methods of processing and testing umbilical cord blood (UCB), effect of various factors (new-born"s weight, height, Apgar scale, gestation time) on collected UCB volume and nucleated cells (NC) quantity 983 UCB units are taken. Each UCB unit in first 24 hours is tested for haemotransmissive infections presence, bacteriological testing is also taken. 122 (12,4%) UCB units are utilized because of infectious danger. Percentage of infected units with microbes-contaminants is 5,8%, contamination from air is 0,1%. Correlative analysis has shown positive dependence of collected UCB volume on new-born"s weight and height. Sadovskaya CHARACTERISTICS OF FLUID-ELECTROLYTIC BALANCE AND THEIR MECHANISMS OF PRETERM INFANTS In the paper a comparative analysis of the most informative parameters of fluid-electrolytic balance used in clinical practice in full-term and pre-term infants is carried out. On the basis of the analysis a necessity of subsequent monitoring the fluid-electrolytic balance in preterm infants for more effective correction of the infusion therapy is argued for. V.A. Children with various forms of USD disease have common appropriateness of immunological process. The comparative analysis of basic therapy has shown positive effect of gold salts not only on a clinical picture of disease, but also on dynamics immunological parameters at children with USD, that allows us to recommend auranofin as the basic therapy. Luzhnova The aim of the paper is to study the quality and quantity changes of the heart electric potentials for the children with arterial hypertension and arterial hypotension. 30 children of 14-16 years old are observed to carry out the abovementioned purpose. Synchronous registration of bioelectric potentials from the surface of the chest at 64 points is performed for all patients. The normal moment cartogrammes for the children with arterial hypertension and arterial hypotension are not revealed. The changes of the heart electric field for children with arterial hypertension as the primary discovery of myocardial hypertrophy are considered. It is found that SD4+ level increases and SD8+,SD16+ levels decrease, IL-6, IL-8, FNO-α levels increase for patients with juvenile idiopathic arthriti (JIA) and patients with juvenile chronic arthritis (JChA) in all children's groups with length of illness more than a year. The concentration increase of TSH, ÒÇ, Ò4 and prolactin content growth in patients with JIA and JChA is found. The activization of inflammatory hormones of the thyroid gland and prolactin which stimulates the inflammatory cytokines and immune system activation is shown be very important in pathogenesis of the disease. Krasnova ULTRASONIC METHOD OF INVESTIGATION OF THE ENDOTHELIUM FUNCTION IN DIAGNOSTICS OF LATE TOXICOSIS An estimation of the endothelial function by the test of reactive hyperemia of brachial artery is given for pregnant females in the third term of physiological pregnancy, for women with toxicosis of light, average and heavy degree of toxicosis. It is found that the vascular dysfunction correlates with the degree of toxicosis gravity. The test can be considerated as an additional criterion for the evaluation of toxicosis gravity and effectiveness of the conducted therapy. M.V. Nechaeva, E.A. Lebedeva IODINE DEFICIENCY, ITS CORRECTION AND EFFECT ON PREGNANCY In the study 241 women living in Samara region have been surveyed at the beginning and in the end of pregnancy for revealing iodine deficiency and determination of their thyreoid status. The obtained data allows us to conclude the deficiency of iodine in Samara region. Pregnancy is the aggravating factor for iodine deficiency, which influences negatively to hestation period. It is found that group iodine preventive therapy reduces the frequency of complications of pregnancy and improves the function of thyroid gland. O.N. Shatunova An analysis of distribution of HLA 1 classes gens and cytokine's concentration in blood serum and peritoneal liquids at patients with chronical salpingo-oophori-tis is carried out. The correlation is enucleated upon of women's illness frequency, severity process of inflammation with certain number of HLA-anti-gens. On account of determination of the concentration of proinflammatory (IL-lα, IL-8) and contrainflammatory (IL-4) cytokines, deep disturbance of immune state on local and common level at pelvic inflammatory disease are determined. This disturbances leading to additional immune modulated therapy and also prognosticate criterions of complication's cultivation and chronic of inflammatory process are proposed. N.A.Krasnova, N.G.Mironova BACTERIAL VAGINOSIS. A NEW METHOD OF VAGINA BIOTOPE TREATMENT 110 patients with bacterial vaginoses are examined. Risk factors of disease are found. We propose a new method of restoration normal vaginal microbial flora by transplantation autolactobacterias. The efficiency of the method is higher than an ordinary treatment. This method is safe and can be used during the pregnancy. O.B. Kalinkina CORRECTION OF INSULIN-RESISTANCE OF WOMEN WITH POLYCYSTIC OVARY SYNDROME Total clinical examination and assessment of hormonal and metabolic states of the patients with polycystic ovary syndrome are carried out. Aberrancy in hormonal background and lipid blood spectrum combined with glucose intolerance are found in those patients. After the appropriate treatment ovulation restored in about 46% of cases of women taking monotherapy with methformine alone versus 76% of women receiving combined therapy of methformine and clostilbegid (clomiphen citrate). Reduced risk of obstetrical complications in women with pre-conceptional correction of the insulin resistance is shown. O.B. Kalinkina An estimation of vaginal biocenosis of women with polycystic ovary syndrome at the preconceptiomal stage is given. 21% of women has the normal type biocenosis, 37% has the intermediate type and 42% with disbiosis. Two-step scheme of treatment including sanitation of vagina with metronidasol, followed by restoration of normal lactobacillus is proposed. For major group of patients auto-lactobacillus which is cultured on the selective media is used to restore the normal vaginal biocenosis. The advantage of this technique compared with simple use of acelact is shown. O.B. Kalinkina PRE-CONCEPTIONAL CORRECTION OF ENDOTHELIAL DYSFUNCTION OF WOMEN WITH POLYCYSTIC OVARY SYNDROME An estimation of gestational toxicosis development in 55 women with poly-cystic ovary syndrome and history of infertility is given. Three pathogenetic variants of toxicosis according to prevalence of cytotrophoblast invasion are found. The results of reactive hyperemia of brachial artery test probed in 46 women with PCOS at the stage of pre-conceptional preparation are considered as a criterion of treatment effeciency. An examination of five probationer without trauma in six series of experiment under control of image tube by our original method is realized. All data are analyzed by ANOVA and Kruskall - Wallis H test. The maximal efficiency of circular plaster bandage from proximal part of forearm to heads of metacarpal bones with immobilization of proximal phalanx I and II fingers in position of "writing capture" is found. Dmitrienko, M.A. Osadchuk 150 patients of young age -30 patients with IDP (chronic gingivitis and parodontitis), proceeding on a background of a peptic ulcer of a stomach (the first group), and also - 30 patients with the specified parodontium's pathology, associated with peptic ulcers of duodenal localizations (the second group) are examined. Groups of comparison includ persons with chronic gingivitis (the third; n=30) and chronic parodontitis (the fourth; n=30) without a pathology of a gastroenteric path. The control group includ practically healthy 20 persons. During inspection endoscopical, histomorphological, immunohistochemical, morphometrical methods are used. Statistical processing is carried out by the aid of the program "Statistica 6,0". IDP on background of peptic ulcer proceed more persistently. One of conducting factors of occurrence and development as IDP, and a peptic ulcer, is Helicobacter pylori (Hp), causing infringement of cellular updating of epitheliocytes a mucous membrane of an oral cavity and a stomach. Eradication Hp at patients with specified associated pathology positively influences process of approach of remission as chronic gingivitis, and chronic parodontitis. T.V. Melenberg, V.P. An injection of polyoxidonium in combination with antiseptic doesn't stimulate it's antimicrobial properties. It also doesn't effect on the microbial resistance to the used medicaments. Malykhina ENZYME THERAPY AS MASTOPATHY PERSPECTIVE TREATMENT Intensive refinement of diagnostic methods results in early diagnostic and augmentation of patients with chronic cystic mastitis. An increase in the number of operative interventions due to of this pathology necessitates the effective method of treatment of the disease. In the paper long experience of conservative chronic cystic mastitis therapy method application is considered. The efficiency of the method at primary treatment rise up to 95% and more, and on 85% decrease the number of operative interventions in mammary gland due to this pathology. T.V. Malykhina SOME PHYSIOLOGICAL ASPECTS OF HORMONES EFFECT ON PROCESSES IN MAMMARY GLANDS In the paper the basic biological processes in mammary gland going under various hormones exposure are considered. The role of different kind of hormones in forming polyferative processes at mammary gland tissue is shown. The necessity of close interaction between oncologists, gynecologists and endocrinologists is discussed. R.I. Solomeina, N.N. Lamash 120 patients with different diseases of mammary glands are observed with magnetic resonance tomography (MRT). The following pathology is detected: diffusive cystic mastitis - 14 cases, nodal mastitis - 37 cases, adenosis -4 cases, cyst — 7 cases, fiberadenoma — 3 cases, malignant affection— 36 cases, no pathology - 19 cases. The diagnosis is confirmed in 101 cases of mammas affection (45 cases based on surgery results, 56 cases based on puncture biopsy results). MRT with contrast enchancement permits to accurately differentiate the nonmalignant and malignant growth of the mammas as well as identifying the origin of axillary lymphatic nodes affection which enlargement can be followed not only by malignant growth but also nonmalignant processes in mammas. The neurological status dynamic is analyzed in late period of patients suffered craniocerebral trauma of different disease heaviness. The results are confirmed by different neuro visualization methods. E.A. Kondrashova, E.P. Krivoshekov, P.A. Lebedev The aim of this study is to predict cardiovascular complications for patients with atherosclerosis obliterans of the lower limb arteries. 115 patients with chronic arterial insufficiency of the lower limb are observed. 35 of them have strain ischaemia (Fontaine stage II) and 80 — critical limb ischaemia (Fontaine stages III and IV). The level of endotoxication is found by the measurement of middle size molecules, plasma creatinine and fibrinogen, leukocyte counts, reaction on C-reactive protein. Cholter ECG monitoring is carried out to reveal cardiac rythm variability, arrythmias and ischaemia periods. These outcomes are confirmed by 93 of observed patients in two year period. The endointoxication parameters have a strong relation to extremity ischaemia and had strong impact on rythm acceleration (depressing its variability) and arrythmogenic effect known to be a predictors of cardiovascular complications. In prospective study the cardial events are determined by endointoxication, age, history of myocardial infarction, chronic heart failure, silent myocardial ischaemia, ectopic myocardial activity. A.V. Dzyubailo The correlation of the nicotine dependence from unremitting length of smoking is studied. The level of the motivation before and after illness at the patients having nicotine dependence with of CO1D (Chronic obstructive lung diseases) is analyzed. Kochetkov VENTRICAL AND DODECADACTYLON PEPTIC ULCER TREATMENT WITH THE HELP OF VAST OZONE AUTOHEMOTHERAPY In the paper the effect of vast ozone autohemotherapy (VOH) on ventricle and dodecadactylon peptic ulcer regeneration is studied. VOH positive effect is supported by clinical, biochemical tests and microscope slide. E.V. Egorova In the paper the efficiency of ultrasound therapy and instillation of the anticataract drugs Oftan-catachrom and Quinax in the prophylaxis of progressive age-specific cataract is studied. Comparative analysis of the crystal picture of patient lacrimal fluid, visual acuity is carried out and the level of lens transparency in the course of treatment using the above-mentioned anticataract drugs is determined. These drugs are used in the form of instillation and in combination with ultrasound therapy. T.V.
